"Just Like Home" Pet Hotel

Agreement & Waiver of Liability & Medical Authorization &Release -

I hereby certify that my dog(s) is/are in good health and has/have not (circle one), been ill with any communicable condition in the last 30 days. All dogs have had flea and tick prevention applied prior to arrival. I further certify that my dog(s) has/have not harmed or shown aggressive or threatening behavior towards any person or any other dog and is friendly to all breeds. (Describe any behavior on the Guest Information Form)

I have read, agree to, and understand the following:

a) Dogs can sometimes receive minor cuts and scratches when in an
environment with other non-family member dogs, and any medical accident that may develop with my dog will be treated as deemed best by staff and helpers
of "Just Like Home" Pet Hotel, at their sole discretion and that I assume full liability and financial responsibility for any and all expenses involved for veterinary services including trip charges.

b) I AUTHORIZE Joan Huff or staff member of "Just Like Home" PetHotel to provide all necessary personal and outside medical services necessary to care for my sick or injured dog. JLH will not give authorization to euthanize.

c) I am solely responsible for any harm or injury my dog inflicts on
any person or other guest staying at "Just Like Home" Pet Hotel,including liability for medical costs, while they are a guest at "Just LikeHome" Pet Hotel.

d) That "Just Like Home" staff has relied on my representation thatmy
dog(s) is/are in good health and has/have not harmed or shown aggressive or threatening behavior towards any person or any other dog.

e) I RELEASE FROM LIABILITY AND HOLD HARMLESS, "Just Like Home"owner, employees, contract staff, agents, officers, helpers, guests, etc., from any claim for loss or injury which may be alleged to have been caused directly or indirectly to any person or thing by the act of my dog. I personally assume all responsibility and liability for any such claim. I also agree to hold aforementioned parties harmless from any claim for loss of my dog by disappearance, theft, and death or otherwise, and any claim to damage or injury to the dog. I also agree to indemnify and save the aforementioned parties harmless from any and all loss and expense, including legal fees.

I certify that I have read, understood, and agree to the above stated
policies.

OTHER IMPORTANT INFORMATION:
1) Accepted payment: VISA, MASTERCARD & ISCOVER/personal check/cash
2) Gift cards not accepted during holiday periods.
3) No discounts during holidays. Special rates will apply for long
term reservations.
4) ** Guest can choose to take the 10% off their total bill or give it
to a Greyhound Rescue group of their choice.
5) Minimum 2 night stay.
6) Pick up by 12 noon.
7) Check in 2 or after.
8) Payment is due at pick-up.
9) 50% Deposit will be required for holidays and reservations over 4
nights.
